Hearts of Mine Community Health, Inc formerly known as Touching Hearts Home Care, Inc, is a for profit agency licensed by the PA State Department of Health, providing exceptional social and health care assistance to address social determinants of health, improve the quality of life among aging populations, and support people over 21 years old with physical disabilities.
